2. Fill-up all the data needed for processing your credit card and submit it.
3. For validation purposes, pls send a copy of your scanned valid ID (Passport or Drivers License) and email it to info@travelph.com OR send a fax to 1-323- 478-1165 (USA) OR 632-4451192 (Manila, Philippines).
4. A confirmation via email or phone will be sent if credit card charging is successfully charged.

3. Fill up all the details and don’t forget your signature.
